D-2-HG IS PRESENT AT STRIKINGLY HIGH LEVELS IN ISOCITRATE DEHYDROGENASE (IDH) MUTANT CANCERS, INCLUDING GLIOMA AND ACUTE MYELOID LEUKEMIA. IT IS WIDELY ACCEPTED THAT D-2-HG ACCUMULATION IS THE INITIAL DRIVER OF TUMORIGENESIS IN THE EARLY STAGES OF THESE CANCERS, AS D-2-HG ANTAGONIZES Α-KETOGLUTARATE-DEPENDENT PROTEINS, CAUSING WIDESPREAD DNA HYPERMETHYLATION. D-2-HG IS PRODUCED AS A DIRECT RESULT OF THE NONCANONICAL ACTIVITY OF CYTOSOLIC IDH1 AND MITOCHONDRIAL IDH2 ENZYMES. MUTATIONS THAT EMERGE IN THE ACTIVE SITES OF IDH1 AND IDH2 BESTOW THE ABILITY OF BOTH ENZYMES TO CONVERT Α-KETOGLUTARATE TO D-2-HG. THERE IS NO DEBATING THAT MUTANT IDH1 AND IDH2 DRIVE MASSIVE INTRACELLULAR CHANGES IN D-2-HG CONCENTRATION. HOWEVER, THE SUBCELLULAR DISTRIBUTION BETWEEN THE CYTOSOL AND MITOCHONDRIA (SITES OF D-2-HG PRODUCTION) AND THE NUCLEUS (SITE OF D-2-HG INTERFERENCE) HAS NOT BEEN ELUCIDATED. CURRENTLY ACCEPTED PRACTICES FOR D-2-HG DETECTION AND QUANTITATIVE MEASUREMENTS INCLUDE LIQUID/GAS CHROMATOGRAPHY COUPLED WITH TANDEM MASS SPECTROMETRY, MAGNETIC RESONANCE SPECTROSCOPY AND BIOCHEMICAL ASSAYS. THESE METHODS ARE PROHIBITIVE FOR DETERMINATION OF SPATIOTEMPORAL DETERMINATION OF D-2- HG CONCENTRATION AS THEY DESTROY THE CELL DURING ANALYSIS. THIS SIGNIFICANTLY IMPEDES THE STUDY OF D-2-HG BIOLOGY IN SITU. OUR RESEARCH TEAM AT NORTHERN MICHIGAN UNIVERSITY ENGINEERED THE GENETICALLY ENCODED FLUORESCENT BIOSENSOR D2HGLO TO ADDRESS THE NEED FOR A TOOL CAPABLE OF SPECIFIC D-2-HG DETECTION IN INTACT CELLS. D2HGLO HAS DEMONSTRATED EFFICACY IN DETERMINING D-2-HG CONCENTRATION IN SUBCELLULAR COMPARTMENTS, ALLOWING US TO INTERROGATE THE ROLE OF D-2-HG IN CANCER PROGRESSION. WE HAVE PRELIMINARY DATA THAT DEMONSTRATES THAT D2HGLO RESPONDS TO PHYSIOLOGICALLY RELEVANT D-2-HG CONCENTRATIONS AND IS TEN-FOLD MORE SELECTIVE FOR D-2-HG COMPARED WITH ITS ENANTIOMER L-2-HG. THE OBJECTIVE OF THIS PROJECT IS TO ELUCIDATE THE MOLECULAR MECHANISMS GOVERNING D-2-HG HOMEOSTASIS IN LIVING CELLS USING THE D2HGLO PLATFORM. THE AIMS OF THIS PROPOSAL ARE TO OPTIMIZE OUR GENETICALLY ENCODED FLUORESCENT BIOSENSOR D2HGLO FOR NONINVASIVE QUANTITATIVE AND SPECIFIC SUBCELLULAR MEASUREMENTS OF D-2-HG IN LIVING CELLS AND TO INVESTIGATE HOW THE MUTATIONAL STATUS OF IDH1 AND IDH2 SHAPES THE SUBCELLULAR D-2-HG LANDSCAPE. UTILIZING ESTABLISHED GLIOMA CELLS LINES AS WELL AS PRIMARY CELL LINES DERIVED FROM PATIENT TUMORS, WE INTEND TO UTILIZE D2HGLO TO INTERROGATE THE SPATIOTEMPORAL DISTRIBUTION OF D-2-HG TO ADVANCE OUR UNDERSTANDING OF D-2-HG DRIVEN ONCOGENESIS. WITH SUCH A POOR CURRENT UNDERSTANDING OF THE CELLULAR DYNAMICS ASSOCIATED WITH D-2-HG PRODUCTION AND LOCALIZATION, D2HGLO IS PRIMED TO ANSWER FUNDAMENTAL BIOLOGICAL QUESTIONS REGARDING THE PLEIOTROPIC ROLES OF D- 2-HG IN CANCER BIOLOGY.

KIRBY RESEARCH CENTER FOR FUNCTIONAL BRAIN IMAGING AT THE KENNEDY KREIGER INSTITUTE FOR THE ANALYTICAL AND CLINICAL VALIDATION OF THE NOVEL, GENETICALLY ENCODED FRET-BASED PROBE, D2HGLO. THE INTENDED USE FOR THIS DIAGNOSTIC AND PREDICTIVE TOOL INCLUDES PREOPERATIVE AND POST-OPERATIVE CHARACTERIZATION OF MUTANT IDH (ISOCITRATE DEHYDROGENASE) GLIOMA THROUGH DETECTION OF THE SURROGATE BIOMARKER AND ONCOMETABOLITE, D-2-HYDROXYGLUTARATE (D-2-HG), WHICH IS THE PRODUCT OF THE MUTANT IDH ENZYME. D2HGLO HAS THE POTENTIAL TO MODIFY CURRENT MUTANT IDH GLIOMA TREATMENT MODALITIES AS THIS DIAGNOSTIC TOOL IS ANTICIPATED TO PREDICT PATIENT RESPONSE TO THERAPEUTIC STRATEGIES MONTHS EARLIER THAN STANDARD MRI ANALYSIS OF DISEASE PROGRESSION. THE 2021 WORLD HEALTH ORGANIZATION (WHO) CLASSIFICATION OF CENTRAL NERVOUS SYSTEM (CNS) TUMORS IDENTIFIES MUTANT IDH GLIOMAS AS A BIOLOGICALLY AND CLINICALLY DISTINCT GROUP, ADDING EMPHASIS ON THE DEVELOPMENT OF AN ASSAY TO CHARACTERIZE THIS CATEGORY OF TUMORS. THE ABSENCE OF AN ASSAY UTILIZING A LIQUID BIOMARKER HAS BEEN A SIGNIFICANT HURDLE FOR DIAGNOSTIC AND PROGNOSTIC CARE OF MUTANT IDH GLIOMA PATIENTS AND THIS SHORTCOMING CAN BE REMEDIED AS D2HGLO SPECIFICALLY DETECTS D-2-HG FROM NON-INVASIVE CLINICAL SPECIMENS. HERE, WE PROPOSE TO CHARACTERIZE THE FIRST LIQUID BIOMARKER ASSAY ALLOWING FOR THE SPECIFIC, INEXPENSIVE, AND NON-INVASIVE DETECTION OF D-2-HG IN URINE, SERUM, AND PLASMA, AS WELL AS CEREBROSPINAL FLUID. OF SIGNIFICANCE TO THIS PROPOSAL IS THE SUCCESS OF THE MUTANT IDH INHIBITOR, VORASIDENIB, A MUTANT IDH 1/2 INHIBITOR. A PHASE III CLINICAL TRIAL FOR THIS THERAPEUTIC AGENT RECENTLY (SEPTEMBER, 2023) DEMONSTRATED CLINICAL UTILITY BY MAINTAINING DISEASE STABILITY AND PROVIDING A DELAY IN THE NEED FOR ADDITIONAL THERAPEUTIC INTERVENTIONS. THE SUCCESS OF VORASIDENIB CAUSED THE TRIAL TO BE HALTED PERMITTING PATIENTS FROM THE PLACEBO GROUP TO BE PLACED ON THE THERAPY AS WELL. WITH FEW SIDE EFFECTS IDENTIFIED, THE SCIENTIFIC AND HEALTHCARE COMMUNITIES EXPECT FDA APPROVAL OF VORASIDENIB AND ITS USE IN TREATING PATIENTS WITH MUTANT IDH GLIOMA. WITH THE SIGNIFICANT ADVENT OF THIS MUTANT IDH THERAPY, D2HGLO IS PERFECTLY POSITIONED TO ADDRESS THE CLINICAL LABORATORY NEED FOR A LIQUID BIOMARKER ASSAY TO DETERMINE IF AND WHEN A PATIENT IS FAILING THERAPY. BY MONITORING D-2-HG PRODUCTION IN PATIENTS WITH MUTANT IDH GLIOMA UTILIZING D2HGLO, CYCLES OF CHEMOTHERAPY ARE MORE LIKELY TO BE ATTENUATED, RESULTING IN BOTH PHYSICAL AND FISCAL RELIEF TO THE PATIENT. IN ADDITION, D2HGLO IS HIGHLY AMENABLE FOR IMPLEMENTATION IN LOCATIONS WHERE TREATMENT RESOURCES ARE LIMITED, THEREBY INCREASING PATIENT ACCESS TO NEEDED HEALTHCARE. THIS PROPOSAL OFFERS VALIDATION OF A FLUID-BASED TEST FOR D-2-HG AND HAS THE POTENTIAL TO BECOME A COST-EFFECTIVE INTEGRATED BIOMARKER TO PREDICT TUMOR TREATMENT RESPONSE AND TUMOR RECURRENCE, THEREBY GUIDING CLINICAL DECISION-MAKING BEYOND THAT ACHIEVABLE USING CURRENT IMAGING TOOLS. ACTIVE LAYER THICKNESS IS ESSENTIAL IN UNDERSTANDING COLD REGIONS AS IT DIRECTLY AFFECTS VEGETATION, WATER FLOW, AND OTHER PROCESSES SUCH AS GROUND STABILITY, WITH IMPLICATIONS FOR COMMUNITY RESILIENCE, INFRASTRUCTURE, NATURAL RESOURCE MANAGEMENT, AND SOCIOECONOMIC DEVELOPMENT. FOR EXAMPLE, KNOWLEDGE ABOUT THE ACTIVE LAYER IS CRITICAL TO BUILDING HOUSES, ROADS, PIPELINES, AND OTHER TYPES OF INFRASTRUCTURE ON PERMAFROST. THE DATA COLLECTED BY THIS PROJECT WILL PROVIDE KNOWLEDGE OF GEOGRAPHIC DISTRIBUTION AND TRENDS IN A FORM THAT CAN BE USED BY DEVELOPERS, ENGINEERS, LOCAL COMMUNITIES, AND PLANNERS TO SUPPORT SMART INFRASTRUCTURE DEVELOPMENT IN ALASKA UNDER RAPIDLY CHANGING ARCTIC CONDITIONS. THESE DATA WILL ALSO HELP TO IMPROVE THE RELIABILITY OF GLOBAL ECOSYSTEM MODELS AND SATELLITE MISSION PRODUCTS. THE CIRCUMPOLAR ACTIVE LAYER MONITORING (CALM) PROJECT HELPS TO COORDINATE AN INTERNATIONAL PERMAFROST NETWORK, CONTRIBUTING TO A WIDELY USED PUBLIC DATABASE. THIS RESEARCH SUPPORTS NATIONAL INTERESTS BY ENHANCING UNDERSTANDING OF THE ARCTIC, PERMAFROST, GEOSTRATEGIC AND INFRASTRUCTURE PLANNING, AND WORKFORCE DEVELOPMENT THROUGH TRAINING STUDENTS. THE CALM PROJECT FOCUSES ON LONG-TERM STANDARDIZED MEASUREMENTS OF ACTIVE-LAYER THICKNESS (ALT) AND DYNAMICS. LOCAL SITE CONDITIONS AND SEASONAL VARIABILITY CREATE COMPLEX INTERACTIONS THAT DETERMINE THE MAGNITUDE OF SEASONAL SOIL THAW AND RESULTING BIOGEOCHEMICAL PROCESSES. TIME SERIES OF THAW MEASUREMENTS AT THE SAME LOCATIONS AND ACROSS TERRAIN TYPES AND REGIONS ARE REQUIRED TO IDENTIFY SCALES OF SPATIAL VARIATION, ESTABLISH TRENDS, AND VALIDATE MODELS. THIS PROJECT MEASURES LONG-TERM ACTIVE LAYER AND GROUND TEMPERATURE AS WELL AS THAW SUBSIDENCE, AT SITES ALONG THREE GEOGRAPHICAL, CLIMATIC, AND ECOLOGICAL TRANSECTS IN NORTHERN ALASKA. DURING THE RESEARCH PERIOD, FURTHER STANDARDIZATION OF THE INSTRUMENTATION AND CHARACTERIZATION OF MACRO AND MICRO-SCALE CONDITIONS AT EACH NORTHERN ALASKAN SITE AND COMPARATIVE ANALYSIS OF THE RELATIVE INFLUENCE OF THESE CONDITIONS WILL BE COMPLETED. THIS PROJECT ALSO SUPPORTS THE INTEGRATION OF ALT, GROUND TEMPERATURES, AND ANCILLARY DATA WITH THOSE FROM INTERNATIONAL PARTNERS (ALMOST 300 SITES ARE IN THE NETWORK) INTO THE GLOBAL TERRESTRIAL NETWORK FOR PERMAFROST DATABASE. RECOMMENDED STANDARD PROTOCOLS FOR SUBSIDENCE MEASUREMENTS AND DATA ARCHIVING WILL BE FINALIZED TO AID IN COMPARISON BETWEEN SITES, LIKE THOSE PREVIOUSLY DEVELOPED FOR ALT AND NEAR-SURFACE TEMPERATURES.
